海角原创 uses a multi-disciplinary approach to treat patients in their later years.聽We help older patients maintain their independence while making sure their medical needs are being effectively addressed.
Geriatrics is a medical specialty addressing the needs of older patients, with an emphasis on coping with aging, chronic disease, and disabilities.聽 Primary care physicians meet the needs of many people over the age of 65, but a geriatrician may be consulted or provide primary care for seniors with complex or multiple medical problems.聽In addition to being certified in either Internal Medicine or Family Practice, geriatricians have earned a Certificate of Added Qualifications in Geriatric Medicine.
海角原创's interdisciplinary approach to geriatrics also includes nurses, nutritionists, physical therapists, occupational therapists, pharmacists, and mental health professionals.聽The geriatrician serves as the point person coordinating the patient's care.聽This team assesses the person's current health and the effects of current or past medical conditions, diseases and injuries.聽They also evaluate the person's ability to perform the activities of daily living, sometimes referred to as ADLs.聽These include bathing, dressing, and eating.聽 The patient's social support, including family, friends, and community groups, is also evaluated with the goal of identifying unmet needs and caregiver support.
